Membership Update Form

Please help the administrative staff update your membership information for 2024 if you have any changes since 2023 forms.  You may download and print to mail or send in a picture attachment to our email listed on the form.

Please share with family and friends who no longer attend and may have moved their membership to another church. In leiu of a form, you may send an email of your intent to move your membership. It is important to notify us if you have moved membership elsewhere so we can have accurate records.  If you have any questions or concerns, please do not hesitate to reach us via the church email.
